“健康申報表”是⾹港常⽤防⽌新冠病毒⾼危⼈⼠進⼊場所、追蹤和通知新冠確診緊密接觸者。⼀般是印刷的表格，但填寫費時失事，花⼤量⼈⼒處理。
“Health Declaration” has been widely used in sports events, concerts, and public places to record and to notify the close contacts with the infected. Most of them are in paper form. A fully automated system should be in place to replace this time-consuming paperwork.
本系統利⽤帶觸控屏幕的終端機，簡化填寫⼿續，也利⽤⼋達通卡的序號，嘟⼀下便減省填寫⼈名的時間。

⼀、流感/新冠症狀?
⼆、在⼗四天內外遊或與新冠病者有密切接觸?
如果兩者答案都沒有，拍⼋達通⼀下確實。數據保留⼗四天，之後⾃動消滅。
